MA/CNA Care Coordinator | New York State

This is a remote role. ChronicCare is seeking organized, compassionate, and tech-savvy Care Coordinators to support our remote Chronic Care Management (CCM) services for Medicare patients. This flexible, part-time opportunity allows you to work from home while playing a vital role in improving the health and well-being of seniors managing multiple chronic conditions. As a Care Coordinator, you’ll serve as the connection between patients and their care teams—providing administrative support, care navigation, and social services coordination. You’ll help ensure patients receive the right care at the right time while avoiding unnecessary ER visits and hospitalizations.

Key Responsibilities:

Use our care management software to call Medicare patients with 2 or more chronic conditions (e.g., Diabetes, COPD, Hypertension, CHF) on a monthly basis

Build rapport and maintain ongoing communication with your assigned patients to support consistent engagement

Collect and update patient information including medications, appointments, symptoms, and social needs

Document every encounter accurately and professionally within the care platform

Identify social determinants of health (transportation needs, food insecurity, housing issues, etc.) and connect patients to appropriate community resources

Coordinate care by assisting with appointment scheduling, specialist referrals, and follow-up care

Encourage patients to complete preventive services such as wellness visits, vaccines, cancer screenings, and lab work

Support Transitional Care Management efforts by helping discharged patients navigate follow-up instructions, appointments, and care needs
Requirements

Fluent in English with excellent communication and interpersonal skills
-
Reliable and self-motivated with the ability to work independently

Strong attention to detail and a passion for patient-centered care

Comfortable using digital tools, web-based software, and electronic documentation

Ability to follow scripts and workflows while exercising sound judgment in real-world patient situations
Education and Experience:
-
High school diploma or GED required
-
1+ year of experience in a healthcare, customer service, or case management support role

Experience in a medical office, insurance, social services, or home health setting is a plus
-
Familiarity with EHR systems and healthcare terminology is preferred
Preferred Education and Experience, but not required:
-
Prior experience in Care Management, Population Health, or Telehealth

Training in Motivational Interviewing or behavioral health coaching

Experience with Medicare patients and documentation standards
Scheduling and Other Requirements

Minimum 15–25 hours/week, Monday–Friday between 9 AM – 6 PM ET

Must have high-speed internet and access to a reliable computer

Work must be performed in a HIPAA-compliant home environment

This is a 1099 contractor position and cannot be combined with a full-time role

You will set and manage your own schedule using our platform
Benefits
Compensation:

$18–$22/hour, based on experience and performance

Pay is based on 3 Clinical Encounters per hour. An encounter occurs when:

A successful patient call is completed, or
-
At least 20 minutes of care coordination is documented in the patient’s chart

Pay Timing:
Monthly via direct deposit, 40 days after the close of each service month, due to Medicare reimbursement cycles. All earnings are guaranteed once the service month ends.
