LPN/RN Nurse | New York State

Pay: $25–$30/hour
Type: 1099 Contractor | Remote | 15–25 hrs/week | Weekdays Only

This is a remote role. ChronicCare is seeking dedicated and tech-savvy Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our team remotely to support Medicare patients through our Chronic Care Management (CCM) program. This flexible, part-time position offers the opportunity to make a real impact by helping patients manage multiple chronic conditions, prevent unnecessary hospitalizations, and achieve better health outcomes—all from the comfort of your home. As a Nurse Care Coach, you will conduct monthly check-ins with patients to provide clinical guidance, care coordination, education, and support, helping them navigate the complexities of chronic disease management.

Key Responsibilities:

  • Use our specialized CCM software to connect monthly with Medicare patients who have 2 or more chronic conditions (e.g., Diabetes, CHF, COPD, Chronic Pain, Hypertension, Depression, etc.)

  • Build strong rapport with patients and serve as a trusted clinical resource and care navigator

  • Deliver individualized coaching and education to help patients understand their conditions, medications, and treatment plans

  • Set and support SMART health goals to promote self-management and improve quality of life

  • Monitor and document changes in symptoms, biometrics, functional status, and medication adherence

  • Collaborate with providers to update and enhance the patient’s Personalized Care Plan

  • Identify and address barriers to care such as transportation, medication affordability, or social support

  • Close preventive care gaps by encouraging and scheduling services like wellness visits, screenings, and immunizations

  • Conduct Transitional Care Management (TCM) calls for high-risk patients recently discharged from hospitals or emergency departments to reduce the risk of readmission

 

Requirements

  • Fluent in English with strong communication and clinical assessment skills

  • Self-starter who can work independently and meet performance goals

  • Passionate about improving patient outcomes through education, engagement, and consistent follow-up

  • Tech-savvy and comfortable using electronic documentation and care platforms

  • Excellent time management, attention to detail, and organizational skills

 

Education and Experience:

  • Active, unrestricted LPN or RN license in New York State

  • 3+ years of clinical experience (home health, primary care, case management, or chronic disease management preferred)

  • Proficiency with EHRs and web-based applications

 

Preferred Education and Experience, but not required:

  • Prior experience in Care Management, Population Health, or Telehealth

  • Training in Motivational Interviewing or behavioral health coaching

  • Experience with Medicare patients and documentation standards

 

Scheduling and Other Requirements

  • Minimum 15–25 hours/week of availability required, Monday through Friday between 9 AM – 6 PM ET

  • Evening and weekend hours not available

  • This position cannot be combined with a full-time job

  • Must have a high-speed internet connection, secure computer, and HIPAA-compliant work environment

  • You will manage your own schedule using our platform

 

Benefits

Compensation:

  • $25–$30/hour, based on licensure and experience

  • Pay is structured around 3 Clinical Encounters per hour. A clinical encounter includes:

    1. A successful clinical call with the patient, or

    2. At least 20 minutes of documented chart activity (e.g., review, coordination, planning)

Pay Timing:

Monthly via direct deposit, 40 days after the end of the service month, due to Medicare processing timelines. Payments are guaranteed at the close of each service month.
