This is a remote role. ChronicCare is seeking passionate, tech-savvy pharmacists to join our team remotely and support patients enrolled in Medicare’s Chronic Care Management (CCM) program. In this flexible part-time role, you will serve as a Pharmacist Care Coach, helping Medicare patients manage multiple chronic conditions while reducing avoidable complications and hospitalizations. You will be assigned a panel of patients to follow monthly via telephonic outreach. As a pharmacist, your expertise will focus on optimizing medication use, identifying and resolving medication-related problems, providing chronic disease education, and promoting preventive care.
Key Responsibilities:

Use our specialized care management software to call Medicare patients with chronic conditions (e.g., Diabetes, CHF, COPD, Chronic Pain) on a monthly basis

Conduct comprehensive medication reviews (CMRs), reconcile medication lists, and identify medication-related issues such as adherence challenges, potential interactions, duplications, or gaps

Provide patient education on chronic disease management, proper medication use, and SMART goal-setting to improve self-management and clinical outcomes

Update and maintain individualized Plans of Care, including documenting medication changes, symptoms, biometrics, care goals, and provider interactions

Help close care gaps by encouraging preventive care (vaccines, wellness visits, screenings) and coordinating specialist referrals

Collaborate with patients' primary care teams as needed and refer to community or pharmacy-based services (e.g., transportation, DME, financial aid for prescriptions)

Conduct Transitional Care Management (TCM) outreach to recently discharged patients to ensure medication understanding and reduce readmissions
Requirements

Fluent in English with strong verbal and written communication skills
-
Self-directed and able to meet performance targets with minimal supervision

Passion for pharmacy’s role in chronic care and improving population health outcomes

Strong critical thinking, problem-solving, and organizational skills

Comfortable with technology, web-based platforms, and electronic documentation
Education and Experience:
-
Active, unrestricted pharmacist license in New York State
-
3+ years of clinical pharmacy experience, preferably in ambulatory care, MTM, or chronic disease management

Proficiency with electronic health records (EHRs) and clinical documentation
Preferred Education and Experience, but not required:
-
Experience in CCM, MTM, or care coordination

Certification as a Board Certified Ambulatory Care Pharmacist (BCACP) or Certified Diabetes Educator (CDCES)

Familiarity with motivational interviewing or patient behavior change strategies
Scheduling and Other Requirements

Minimum 15–25 hours/week availability, day, evenings, and weekends available

Must have a reliable, high-speed internet connection and secure, HIPAA-compliant workspace

You will schedule your own availability within our platform

1099 contractor role – you are responsible for your own taxes and insurance
Benefits
Compensation:
-
$35–$40/hour, based on performance and experience

Pay is based on 3 Clinical Encounters per hour. An encounter is defined as:

A successful clinical call with the patient, or
-
At least 20 minutes of documented medication reconciliation in a patient’s chart

Pay Timing:
Monthly via direct deposit, 40 days after the end of the service month (to allow for Medicare claim processing). Monthly pay is guaranteed once the month is complete.
