Use your EHR data tools to find eligible (Medicare) patients with chronic conditions (ICD-10).

Step 2: Enroll:
We provide your pharmacy with rack cards and bag stuffers. Give them to eligible patients at point of sale.
We also help you communicate the benefits of CCM to your patients and obtain their consent.

Step 3: Care Management
Our virtual care team conducts a monthly 20-minute clinical check-ins with your enrolled patients.
Our calls are focused on medication reconciliation, updating care plans, lifestyle support and addressing social determinants of health.

Step 4: Bill CMS
Automated documentation and time tracking ensures we meet all compliance hurdles for easy monthly billing.

Your pharmacy will earn administrative service fees each month for each of your patients enrolled.

Dr. Sarah J., Pharmacy Owner
"Adding CCM was the best decision we made this year. Not only did we see a $5000 increase in monthly revenue within 90 days, but our diabetic patients have seen a measurable improvement in their A1c levels."

Mark T., Lead Pharmacist
"Our patients love the extra attention. They feel like the pharmacy actually cares about their health, not just their prescriptions. It has boosted our local reputation immensely."

Dr. Adil M., Pharmacy Owner
"For years, we were stuck in a cycle of 'dispense and repeat.' Since implementing this CCM program, Our patients feel like I'm their partner in health, not just their pill provider. It's the most fulfilling-and profitable-shift we've made in a decade."
Frequently Asked Questions
What is CCM & how does it help pharmacies?
Chronic Care Management (CCM) is a CMS initiative to support Medicare patients living with chronic conditions.
CMS requires a minimum of 20 minutes of non-face-to-face care coordination per month. Pharmacies who participate can benefit financially, stand out from the competition, support their patients, and improve their Medicare Star Ratings.
Do I need to hire additional staff for this?
No. Partnering with ChronicCare means your pharmacy gets a turnkey solution for CCM. This means your team can focus on introducing the program to eligible patients, our virtual care team will be responsible for providing the service each month.
Can smaller independent pharmacies participate in CCM?
Yes. Most pharmacies reach the break-even point with as few as 30 enrolled patients. Beyond that, the revenue scales quickly. Not to mention CCM can improve the pharmacy's Star Ratings and the clinical outcomes of your most complex patients.
What if a patient is already enrolled in CCM through their doctor?
CMS only allows one provider to bill for CCM (CPT 99490) per patient, per month. If a patient is already enrolled elsewhere, you have two choices: do not enroll them in your program, or speak with the patient and their doctor about why a pharmacist-led model is more beneficial for them.
Will this interfere with pharmacy workflow?
No. ChronicCare offers a full-service, pharmacist-led model. We build a care team around your enrolled patient populations, providing clinical touch points on a monthly basis. Your workflow largely remains the unchanged.
Is this different from MTM?
Yes. While complementary, Chronic Care Management (CCM) is a distinct service billed monthly for ongoing care coordination of chronic conditions, whereas Medication Therapy Management (MTM) is often a yearly or episodic review.
